Charitable Donations

The AlohaCare Foundation also offers charitable contributions for entities whose requests may not fall into the categories of grant or sponsorship funds. In keeping with our belief in whole-person care, priority will be given to organizations that work to improve the health and well-being of Hawaiʻi’s most underserved populations. For the purpose of our charitable donations, whole-person care can include addressing mental, physical, and spiritual health as well as social determinants of health such as access to housing, education, and nutrition security, among others.

Application Process

STEP 1: Submit Form

Applicant submits complete online application form below, including an IRS W-9 Form.

STEP 2: Application Review

Application is reviewed for completeness.

STEP 3: Follow Up

The AlohaCare Foundation may request additional information and/or a meeting.

STEP 4: Confirmation of Charitable Donations

The AlohaCare Foundation will provide written confirmation via email to confirm the charitable donation and coordinate details.
